你一定聽過「雙劍合璧威力大」，今天主角是兩把專治腎臟與心臟的小利刃──Finerenone 與 SGLT2 抑制劑（以下簡稱 SGLT2i）。兩者都是慢性腎臟病（CKD）＋第二型糖尿病患者的救星，但其中一把（Finerenone）用得太猛，血鉀可能飆高，讓醫師跟護理師捏把冷汗。於是大家好奇：如果把 Finerenone 與 SGLT2i 合併，血鉀會不會比較乖？
Finerenone：溫柔護腎的同時，也愛「藏鉀」
Finerenone 是最新一代的選擇性醛固酮受體拮抗劑，能減輕腎臟與心臟的纖維化、發炎反應。可惜它像個「愛收集鈉鉀點數」的阿嬤，常常把鉀離子留在血中，久了就可能導致高血鉀。
SGLT2i：邊省糖邊放水，順手帶走多餘的鉀
SGLT2i 原本是降血糖藥，卻意外變成護腎護心的「外掛」。它的工作像在腎小管裝一個「出水口」，把多餘的葡萄糖和鈉帶走，還順便帶走水分，讓病人尿多一點、血壓掉一點。最重要的是，它不太留鉀，甚至有研究說某些族群血鉀還會略降。

Finerenone：高血鉀風險↑，用藥時得三不五時抽血盯著看。
SGLT2i：血鉀風險 ≈ 正常，甚至有「微降」的潛力。

想上 Finerenone？ 先確認血鉀 < 4.8 mmol/L，再安排定期抽血。
已在用 Finerenone，擔心血鉀？ 考慮加上 SGLT2i，不只能顧腎顧心，還可能幫忙「稀釋」高血鉀風險。
合併療法≠免監測：再怎麼安全，CKD 患者仍需定期檢查腎功能與電解質；飲食中高鉀食物也得適量。
臨床指南持續更新中：等 CONFIDENCE 等大型試驗結果出爐，或許未來「Finerenone＋SGLT2i」會變成標配。
一句結論
把 SGLT2i 當後盾，Finerenone 不再是孤軍奮戰；對 CKD＋糖友來說，這對雙劍合璧有望守護腎臟，同時把高血鉀風險降到更低。
